Otto Bremer Trust awards $50,000 grant to The Salvation Army La Crosse County

LA CROSSE – A $50,000 grant awarded by the Otto Bremer Trust will allow The Salvation Army of La Crosse County to add a Community Health Worker position to their Social Sercies Team. The Community Health Worker focuses on the Collaborative Health & Wellness Service (CHWS), which is based on the triad of an individual’s physical, mental, and spiritual needs.

Since 2016, The Salvation Army of La Crosse County has been cultivating a collaboration with local hospitals, the county, and the universities to meet the merging need to provide a more effective and efficient way for individuals/families to receive physical health, mental health, and preventative services. To better serve the needs of the population and community, The Salvation Army organization completed a Community Needs Assessment in 2017 to evaluate its programs/services. One specific focus of the needs assessment was on a newly launched TSA program housed within the doors of the shelter intake system – the Collaborative Health & Wellness Service (CHWS), which consists of partners from the La Crosse County Health Department, Gundersen Health System, Mayo Clinic Health System, and Viterbo University.

“The heart of the CHWS model is the Community Health Worker,” says Social Services Director Krista Coey, “A Community Health Worker’s duty includes coordination with Shelter Staff, Case Managers, external partners, and the CHWS team.”

A press conference regarding the new position will be held on Monday, June 13th at 11am featuring the Collaborative Health & Wellness Service partners as well as celebrate the gift of the Otto Bremer Trust at The Salvation Army of La Crosse County’s administrative offices located at 223 N 8th Street.
